Faith Hospice is a community-based hospice devoted to clinical excellence, emotional support, and spiritual well-being.  Our program is licensed by the State of Oklahoma and is certified by Medicare.  Faith Hospice provides services through the collaborative effort of doctors, nurses, social workers, home health aides, therapists, clergy, trained volunteers, and many others.

WHAT TO EXPECT FROM FAITH HOSPICE

• Pain management

• Medications related to the terminal illness, including those for pain and related symptoms
• Emotional and spiritual support
• Trained volunteers

COMMON HOSPICE DIAGNOSES

Common hospice diagnoses include cancer, heart disease, lung disease, ALS, dementia, liver disease, Parkinson’s, renal disease, stroke/coma, multiple sclerosis, debility unspecified, and failure to thrive.

WHO MAY MAKE A REFERRAL

Anyone may make a referral, including physicians, family, and friends.
